Cover stamped UL/CSA. Knurl nut assembled. Four 6 inch leads, 18awg AWM/TEW 105c 600v plastic, two black and two blue leads, free ends stripped 3/4 inch. Leviton # 382 Pre-Heat Fluorescent Push Button Switch � 5� Leads 2-Black 2-Blue � Rated 40W-125V � Push and hold while fi xture starts, once it is on, release the switch and it stays on.
